Project Description:
Jan. 1st, 2023 brought a statewide systems change to Michigan Medicaid. Medicaid recipients are now eligible to access doula services as a preventative care mechanism. Medicaid will reimburse doulas for up to three prenatal visits, attendance at labor and delivery, and up to three postpartum visits in support of improving maternal & infant health outcomes for pregnant individuals & their families. The GSC has invested significant funds into bringing a DONA International Birth Doula training to Boyne City to reduce barriers for individuals entering the field of birthwork and to ensure equitable access to local doulas for pregnant individuals & their families. With funds being allocated to birth doula training, we'd like to expand into ensuring these individuals also have access to high-quality postpartum doula training to ensure integrated systems & continuity of care. Funds would be used to support scholarships for doulas who otherwise may not be able to a attend Postpartum training.

If your program serves both males and females, how will Hestia funds specifically target women and girls?
Birthworkers are most often female and all current applicants to the birth doula training are female. Pregnant individuals are most often female. The babies born to these individuals may be male or female and do make up part of the 70 people we will reach, as those babies will benefit from these services.
How are recipients identified or selected to receive services? Is financial need considered?
We created a doula training application which asks applicants to commit to serving a *minimum* of two Michigan Medicaid Recipients each year, in addition to any private pay or private insurance recipients they may wish to serve. All doula training applicants commit to serving individuals & families of all race, color, religion, age, sex, national origin, sexual orientation, gender identity or expression, & abilities. Applicants must commit to becoming a Michigan Dept. of Health & Human Services Certified Doula, joining the MDHHS Doula Registry & enrolling as a Medicaid provider which will ensure their ability to receive Medicaid reimbursement and their placement on the statewide search feature, allowing Medicaid Recipients to locate & contact them. Applicants have an opportunity to make a personal investment into their training expense, with canned breakdowns in percentage of training cost ranging from full expense is self-funded to requesting a full scholarship due to financial need.

How will this project meet Hestia's mission statement?
The Great Start Collaborative has four key outcomes, the first is Children are Born Healthy. We hope to operationalize this statewide systems change, by ensuring local access to doulas to improve infant and maternal health outcomes and increase the likelihood that children are born healthy and families are supported in the very early days of family growth and expansion. Benefits of working with a postpartum doula include support & assistance with planning & making informed choice based on personal values, connecting families with additional resources & support, & facilitating effective communication between families & the healthcare team. The emotional support provided by doulas has proven to reduce anxiety, increase confidence, boost self-esteem, improve postpartum mental health, & enhance family bonding & support for partners & siblings. The physical support provided has proven to improve relaxation, enhance comfort & reassurance & improve lactation support. Additionally, these new birthworkers will have career opportunities that will propel their economic status, allowing them to better support themselves, & their families. Having a supportive community of birthworkers and birth supports weaves women and girls together.
How will you evaluate or measure your success:
Post training data collection will occur. We will gather information regarding the number of postpartum doulas who completed training and are serving in Charlevoix & Emmet Counties, the number who then went on to become MDHHS Certified Doulas on the Doula Registry, the number of families they have connected to and serviced via postpartum visits/supports and gather feedback about additional supports, trainings, etc. that may be needed to ensure these doulas are competent, capable, and prepared to serve families and build a sustainable business model for themselves. In total, 14 new postpartum doulas will complete training and service Charlevoix and Emmet county residents.
